Job Description

Responsibilities:
1. Acts as a liaison to the performance improvement activities of the nursing department. 2. Assists the Nursing Leadership of each department in formulating the goals and objectives of the annual performance improvement plan. 3. Assist in the implementation and evaluation of the PI plan. 4. Revision of the PI plan based on the appraisal of the PI activities of the department during the previous year. 5. Monitors adherence to the standards of care and practice related to patient occurrences e.g. falls, elopements, code blue, etc. 6. Reviews occurrence reports, generates memos to specific nursing service leaders, department heads for any issue identified. Evaluate response for appropriateness. 7. Coordinates/aggregates reviews done on patient focused and organizational functions with specific indicators such as: Care of patients on restraints, patient falls, medication administration, etc. 8. Participates in committees such as Central Nursing Performance Improvement, Medication Issue Committee, records and submits minutes of the meeting. 9. Reviews and analyzes monthly and quarterly improvement activity reports of each service for discussion in the monthly CNPI meeting. 10. Develops tools necessary to monitor compliance to the different standards of care to ensure and improve quality of care. 11. Assist in the preparation of reports for HHC Board, Quality Council and other quality management related issues. 12. Maintains confidential records of responses to patient care issues and reports on performance improvement activities 13. Assist in implementing on-going staff education program regarding IOP program in collaboration with other disciplines. 14. Random clinical rounds/ medical records review to ensure compliance with standards of practice and care. 15. Performs other duties responsibilities based on the need of the Nursing Department.

Minimum Qualifications

1. Licensed and currently registered to practice as a professional nurse in New York State, plus (2) or (3), below. 2. Masters Degree in Nursing Administration, Education, Clinical Specialty or equivalent field and five years of satisfactory nursing experience, of which three years shall have been in a supervisory or teaching capacity; or a Baccalaureate Degree in Nursing and six years of satisfactory nursing experience, four years of which shall have been in a supervisory or teaching capacity. 3. A satisfactory equivalent of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.
